A Note From Your School Nurse...
STUDENT WELFARE The health and welfare of each every student is our number one concern. In order to provide quality health services for your child we need your help in keeping us informed of any vital information that pertains to your child and his learning needs while at school. You will need to inform your school nurse and your child's teacher of any health concern which may influence your child's school performance. These conditions may include, but are not limited to; physical disabilities, vision or hearing impairments, seizures, diabetes, heart conditions, migraine headaches, asthma, allergies, kidney disorders or attention needs.
If you wish your child to receive medication during school hours, you are required to complete and sign a Medication Permission Form available at the school office.
When your child is enrolled, all relevant information is recorded, but should your change your address, phone number or place of contract, please notify the school immediately to enable office staff of contact parents as soon as possible in the event of an accident or illness.
In case of accident or illness, only basic first aid can be administered. Parents will be contacted as soon as possible and advised of the circumstances, usually with the recommendation that the child be collected and taken home
INFECTIONS DISEASES
PLEASE DO NOT SEND SICK CHILDREN TO SCHOOL
Children who contract an infectious disease are to be kept at home until they are fully recovered. Parents and Guardians are advised to contact the school office if their children contract any of the following diseases, as various exclusion periods and rules apply.
Chicken-pox, Shingles, Diphtheria, Whopping Cough, Measles, Mumps, Rubella, Viral Hepatitis, Tuberculosis, Poliomyelitis, Typhoid, Streptococcal Infection,
Conjunctivitis, Hand, Foot and Mouth Disease, Impetigo, Head lice.
IMMUNIZATIONS DATES NEEDED!
The Wisconsin Immunization law (statue 140.05) requires the following immunizations for the 2003-2004 school year for all students:
A fine of up to $25 per day or exclusion from school may be imposed for non-compliance with the law by the 30th school day.
Parents please complete your child's immunization record and return it to the school office as soon as possible. Our school district is required by law to report all non-compliant students to the district attorney.
CHECK AHEAD!
Before returning to school, be sure to check your children's heads for those pesky head lice.
They are usually found on the scalp behind the ears and near the neckline at the back of the neck. If you find them don't panic just treat the infestation - contact your school nurse or the local public health department if you need help.
Head lice infestations are spread from the community and into the schools. In particular, elementary school children are the group most likely to contact head lice. It's a common problem in schools worldwide. 6 to 12 million people may be affected yearly.
From time to time infestations of head lice occur at our school. A child found to have active head lice infestation will be sent home and must be treated before returning to school. It is our policy to inform the rest of the class and ask parents to inspect their children and treat them accordingly. The letter sent home to parents contains a useful guide to treatment.
VISION AND HEARING SCREENING
Wisconsin law requests each student entering kindergarten have a eye exam by a physician or an optometrist. In the first few weeks of October, vision and hearing screening will start for grades 1-5, (the kindergartners were screened last fall when prescreening was done for school entrance). Screening is done annually for all students to detect and treat promptly any potential or serious health problems. If a problem is detected, the parent will be notified and a physician referral and referral follow-up will be done to assure intervention. We are looking for parent volunteers to assist us in the screening process. If you are interested in helping out, please call the school office.
Walworth County Public Health Immunization Clinics
ØDTaP, Td, TdaP, Polio, Hib, MMR, Hepatitis B, Hepatitis A, Chickenpox, Rotavirus, Pneumococcal Conjugate and Meningococcal vaccines are available at all clinic sites. Must bring written immunization record to be eligible for vaccination.
ØTB skin testing is available at the Tuesday clinics. Must come to Elkhorn in 48-72 hours to have the skin test read.
ØSpanish interpreters are available at all clinics
ØThere is a small administration fee, no charge for MA/BadgerCare recipients. No one is denied vaccine for failure to pay.
ØChildren under age 18 must have a parent or legal guardian present or have written permission to be vaccinated.

Ø Las pruebas de la tuberculosis están disponibles en las clínicas de los martes. Usted debe de venir a Elkhorn después de 48 - 72 horas para que determinemos los resultados.
